1.Describe your product and its intended use. 描述產品以及用途
List models, types or series. Outline any similarities & differences between products. 列出產品型號/序列號，概述每一種產品的相同之處于不同之處。

2. Identify each of the products components and materials by: 列出每種產品的構成與原材料
A - Manufacturer's name 工廠名稱
B - Catalog number, size, rating 產品說明書號碼、大小尺寸、級別
C - UL Listing or Recognition 是否有過UL的鑒定和推介，如果有，請列出來
D - Polymeric material used, with an explanation of usage and pertinent data (generic name, manufacturer & type designation) 所用的聚合材料的明細和他們的使用說明以及相關的數據（總的原料名稱、供貨的工廠名稱、設計類型）

3. Include if available: 如果有可能的話，請提供更多的材料：
A - Schematic diagrams illustrating electrical circuitry 有圖解說明的電路原理圖
B - Photographs or sketches 照片或者素描
C - Sales brochures or catalogs 銷售使用的說明書
D - Manuals, operation/installation instructions 使用指南、操作說明書、組裝說明書等。

4. Describe, if available, any alternate construction(s) planned for future production. Include alternate parts, materials or arrangements of parts. 如有可能的話，請描述將來的替代產品，包括未來需要改進的某部分的零部件，原材料，以及零部件的新型組合

5. Include name, address, telephone, e-mail and fax number of the: 提交者的名字、地址、電話、傳真、電子郵件箱等聯絡資料：
A - Applicant (Please provide Federal Tax ID number if located in US) 提交者（如果是美國公司，請提交稅務號碼）The company who requests the investigation and will be responsible for payment of invoices 提交者就是提出申請者并負責付款的公司
B - Listee 登記者
The company whose name will appear in our published records and product directories 登記者就是該企業的名字出現在我們的公開紀錄和產品導航欄目里面的公司
C - Manufacturer(s) of the product 制造廠名稱
